Bilateral proliferative retinopathy with blood-tinged hypopyon as the first manifestation of chronic myeloid leukemia

A 36-year-old healthy man presented with bilateral decreased vision and eye pain for 2 weeks. Fundus examination showed vitreous hemorrhage and neovascular frond (OD) [Figure 1]a and tortuous retinal vessels, retinal hemorrhages, neovascularization, cotton wool spots, and vitreous hemorrhage (OS) [Figure 1]b.[1],[2] Slit-lamp examination showed anterior uveitis with rubeosis iridis and blood-tinged hypopyon (OD) [Figure 1]c, [Figure 1]d and white hypopyon (OS) [Figure 1]e.[3] Peripheral blood smear showed hyperleukocytosis with myeloid left shift, thrombocytosis, 16% metamyelocytes, and 3% myeloblasts. Cytogenetic analysis was positive for the Philadelphia chromosome in all metaphase cells. Bone marrow aspiration cytology revealed hypercellular bone marrow with features consistent with chronic myeloid leukemia.
