Large clinically-significant macular edema in a vitrectomized eye with history of macular hole surgery

A 55-year-old female presented with blurring of vision in the left eye. She was a known case of diabetes and had undergone macular hole surgery in the same eye 6 months back (sequelae of clinically significant macular edema (CSME)). In diabetic retinopathy with macular edema, macular holes may develop because of intraretinal exudation combined with increased vitreomacular attachments and tractions.[1]

On examination, her best corrected visual acuity (BCVA) was 4/60 in the left eye. Optical coherence tomography (OCT) macula showed a large CSME measuring 1339 μm × 2554 μm [Figure 1].

This type of a large CSME in a vitrectomized eye with history of internal limiting membrane (ILM) peeling is rare. A recent onset of uncontrolled diabetes might have contributed to this.
