Solitary pigmented “anterior” vitreous cyst

An asymptomatic 24-year-old male was incidentally found to have a cyst in the anterior vitreous in the right eye [Figure 1]a, [Figure 1]b, [Figure 1]c. The cyst was pigmented, loculated, and clear with positive transillumination [Figure 1]b and [Figure 1]c. The cyst was freely floating, with excellent after-movements, and had no attachment to any other ocular structure. Solitary vitreous cysts are rare entities thought to arise from pigment epithelium usually found in the posterior vitreous.[1] The cyst in our case was way too anterior and may have arisen from iris or ciliary body. | Figure 1: (a) Retro-illumination slit-lamp photograph ×16; (b) retro-illumination slit-lamp photograph ×25; (c) oblique illumination slit-lamp photograph ×25
