Subconjunctival silicone oil: Perils of suture-less vitrectomy

Silicone oil is one of the key adjuncts even in modern microincision vitrectomy surgeries (MIVS). While several of its complications are well known, one complication that is specific to MIVS is the subconjunctival migration of oil, especially with high myopes and nystagmus cases., This may range from small-sized [Figure 1]a to medium-sized [Figure 1]b silicone oil bubbles in the subconjunctival space, which at times can be massive too [Figure 1]c. These subconjunctival oil bubbles may lead to chronic inflammation and conjunctival scarring and defeat the very purpose of MIVS. Therefore, one should have a low threshold for suturing the sclerotomies in MIVS, especially when using silicone oil as tamponade.
