A monocular high myope, post-trabeculectomy 8 years ago developed macular hole-related retinal detachment. She underwent pars plana vitrectomy with silicon oil (SO) tamponade. Post-operatively due to extensive subconjunctival SO migration via trabeculectomy ostium, there was an intractable increase in intraocular pressure (IOP).[1] This diffuse-illumination slit-lamp image shows subconjunctival SO in the area of the bleb and an early superonasal staphyloma at the sclerotomy site. The subconjunctival SO gives a bubble-wrap appearance [Fig 1]. Following SO removal, the retina was attached and IOP was stabilized with antiglaucoma medications. SO migration via sclerotomy[1],[2] or necrotic sclera[3] has been noted.
Figure 1: Subconjunctival SO in the area of the bleb (a) and early staphyloma at the superonasal sclerotomy site (asterisk). (b) Peripheral iridectomy (white arrow) and SO in the area of the bleb and temporal subconjunctival space giving a bubble-wrap appearance.
