A monocular high myope, post-trabeculectomy 8 years ago developed macular hole-related retinal detachment. She underwent pars plana vitrectomy with silicon oil (SO) tamponade. Post-operatively due to extensive subconjunctival SO migration via trabeculectomy ostium, there was an intractable increase in intraocular pressure (IOP).[1] This diffuse-illumination slit-lamp image shows subconjunctival SO in the area of the bleb and an early superonasal staphyloma at the sclerotomy site. The subconjunctival SO gives a bubble-wrap appearance [Fig 1]. Following SO removal, the retina was attached and IOP was stabilized with antiglaucoma medications. SO migration via sclerotomy[1],[2] or necrotic sclera[3] has been noted. | Figure 1: Subconjunctival SO in the area of the bleb (a) and early staphyloma at the superonasal sclerotomy site (asterisk).
