Capsular neovascularization subsequent to silicone oil–induced pupillary block

Anterior ocular neovascularization may develop in eyes with chronic anterior segment ischemia, vascular occlusive retinal pathologies, peripheral retinal detachment, or long proliferative vitreoretinopathy.[1],[2] In such cases due to ocular ischemia, angiogenic factors are produced that diffuses from the vitreous to the anterior chamber, resulting in neovascularization of the iris and lens. Neovascularization of the crystalline lens in pediatric patients have been rarely reported in literature.
A 13-year-old boy presented to us who had history of left eye scleral buckling with endolaser accompanied by membrane peeling and pars plana vitrectomy with silicone oil injection done under general anesthesia. After 3 months since initial surgery, he developed silicone oil–induced angle-closure glaucoma, complicated cataract, and silicone oil in anterior chamber along with capsular neovascularization (NVC).
Slit-lamp biomicroscopy showed a mydriatic pupil with silicone oil blocking pupil, absence of neovascularization of the iris (NVI) and total cataract with a fine circumferential radiating spoke-like net of anterior capsular vessel over 360° [Figure 1]. Our case had NVC in the absence of neovascularization of the iris which is not commonly seen in pediatric patients.
