A 65-year-old male presented with complaints of glare in LE. He had undergone phacoemulsification with diabetic vitrectomy 10 years back in LE followed by Nd:YAG laser capsulotomy for posterior capsular opacification (PCO) three years later. Slit-lamp examination revealed the presence of Elschnig pearls at the capsulotomy margin (white arrows), extending onto the posterior surface of the posterior capsule [Figure 1]a and [Figure 1]b, confirmed on anterior segment optical coherent tomography. Elschnig pearls have been known to form around capsular margin and may spontaneously regress. Capsulotomy of a larger size might be considered in post-vitrectomized eyes due to risk of recurrence of PCO.,
|Figure 1: Clinical photograph of LE (a) showing a ring-like arrangement of Elschnig pearls. These are likely formed by the swelling of lens epithelial cells around the capsulotomy margin (white arrows), well-illustrated on retro illumination (b)|
