A 65-year-old male patient presented with nonimprovement of vision after cataract surgery. On examination, anterior segment showed aphakia and two rigid polymethylmethacrylate intraocular lenses (IOLs) inside vitreous cavity lying on retina [Figure 1]. The patient underwent pars plana vitrectomy, IOLs explantation with scleral fixation of IOL.,

Such cases have been reported where even after dislocation of one IOL intraoperatively, another IOL implantation was attempted. Such attempts should be avoided. Even with successful implantation of one IOL, already posteriorly dislocated IOL needs to be removed. Such dislocated IOLs are removed through anterior segment which needs explantation of second IOL too. So to avoid unnecessary surgical burden, such practice of second IOL implantation after one IOL dislocation should not be done.
