Blue illusions on slit-lamp microscopy through yellow intraocular lens explained

Blue/violet illusional slits are often visualized during slit-lamp examination of a pseudophakic patient with hydrophobic acrylic yellow intraocular lens in situ [Figure 1]a. The reason for this appearance is selective reflection of lower wavelength blue light by the yellow intra-ocular lens (IOL).[1] We support the hypothesis based on the fact that blue reflection is similarly seen when white light is shone across blue filtering glasses [Figure 1]b. | Figure 1: (a) Oblique slit-lamp illumination showing yellow IOL in situ with illusional blue slits (arrow); (b) light from indirect ophthalmoscopy shone across blue filtering glasses – blue light reflected back (arrow)
