Sunset intraocular lens dislocation and imprinted anterior capsule

A man in his forties, operated elsewhere for cataract 5 years back, presented with sudden diminution of vision. Torchlight examination revealed a jet-black pupil but the intraocular lens (IOL) margins could be seen. A slit-lamp examination revealed aphakia with dislocation of the IOL looking like a setting sun. Interestingly, the margins of the IOL were imprinted on the anterior capsule mimicking the lens on torchlight examination [Figure 1]. | Figure 1: (a) Diffuse slit-lamp illumination showing jet-black pupil (aphakia) with margins of the IOL imprinted on the anterior lens capsule. Late spontaneous posterior capsule rupture has been previously documented.[1] Imprinted anterior capsule points toward the migration of lens epithelial cells along the edges of the implant. Later metaplastic changes which inherit fibrocontractile property might be the cause of stress leading to the posterior capsular rupture.[2]
