A 66-year-old healthy male presented with visual acuity of 6/60 in the right eye (RE). Fundus [Figure 1]a revealed two optic disc pits (ODP; white arrows), inferior choroidal coloboma with surrounding laser scars (black arrow), and a full-thickness macular hole (MH; blue arrow) in the RE, which was confirmed on OCT [Figure 1]b. Following vitrectomy, ILM peeling, and gas tamponade, he regained visual acuity of 6/18 with type-1 MH closure [Figure 1]c. Absence of other signs of ODP maculopathy and age of patient suggest MH to be of idiopathic origin. Very few cases have reported MH in association with ODP or fundal coloboma.,, This case is a very rare presentation with three different pathologies in the same eye.
|Figure 1: Right eye fundus imaging (a) showing lasered type-3 choroidal coloboma (black arrow), two optic disc pit (white arrow), and macular hole (blue arrow). Optical coherence tomography (OCT-b) showing full-thickness macular hole with cystoid spaces at the margin. Postoperative closure of macular hole was seen on OCT (c)|
