Blue bleb – Post-phacoemulsification retained trypan blue dye in an old bleb

A 68-year-old lady with primary angle-closure glaucoma presented with defective vision in her left eye in which she had undergone trabeculectomy 10 years back. Her UCVA in OS was 20/100 with an intraocular pressure of 18 mm Hg by Goldman applanation tonometer. Slit-lamp findings showed diffuse, encysted, avascular bleb and cataract of nuclear sclerosis grade 4 for which phacoemulsification with foldable intraocular lens implantation was performed. To enhance the anterior capsular visibility, staining was done with 0.1 ml of 0.06% trypan blue dye,[1] which was seen entering the bleb. On the first postoperative day, diffuse blue staining of the filtering bleb was noted [Figure 1], which reversed to normal color at one-day interval.

Conclusive statement: Trypan Blue Dye can be used for confirming the patency[2] of bleb and rule out encapsulation.
