Self-resolving cataract - “Lens feathering“

A 15-year-old male with rhegmatogenous retinal detachment underwent pars plana vitrectomy, fluid air exchange, endolaser photocoagulation, and silicon oil injection with encircling band. On the first post-operative day anterior segment showed feathering of the pre-operatively clear lens [Figure 1]a and [Figure 1]b. Further enquiry revealed poor compliance to the advised prone positioning. One week later, after maintaining prone position, the lens changes had resolved [Figure 1]c, and [Figure 1]d. Lens feathering occurs due to lens dehydration[1] and alteration in metabolism after internal tamponade with gas, air, or silicon oil.[2],[3] It is self-resolving, but may require surgery in case of progression.
