A 22-year-old male presented with defective vision in the left eye (LE) for 2 months duration. Corrected distance visual acuity (CDVA) in LE was 20/250. On examination, both eyes revealed posterior polar cataract (PPC) with posterior capsule (PC) dehiscence in LE [Figure 1]a and [Figure 1]b. The fundus examination was unremarkable. The patient successfully underwent phacoemulsification with hydrophobic acrylic intraocular lens (IOL) implantation in LE with good postoperative visual outcome. The uniqueness of this image is that it mimics an eye inside an eye. In such patients, proper preoperative workup and appropriate surgical management will result in a good visual outcome.[1],[2] | Figure 1: (a) Slit-lamp photography using diffuse illumination showing PPC with preexisting PC dehiscence. (b) Retro illumination photograph of the same eye as shown in Figure 1a
