Macular hole in combined hamartoma of the retina and retinal pigment epithelium

A 40-year-old male complained of outward deviation of the right eye for 3 years with a best-corrected visual acuity of counting fingers at 1 m in his right eye. Fundus examination revealed a dull-gray macular lesion, epiretinal membrane (ERM), changes in retinal pigment epithelium, and hairpin loop-shaped retinal vessels surrounding the lesion [Figure 1]a and [Figure 1]b. Cross-sectional optical coherence tomography (OCT) revealed disorganization of retinal layers, presence of ERM, and a near full-thickness macular hole ([Figure 1]c, white arrow). Combined hamartoma of the retina and retinal pigment epithelium with a macular hole[1],[2] was diagnosed and the patient was counseled and managed conservatively.
