Beware of the bruise: Eyelid and periocular cutaneous angiosarcoma

A 55-year-old immunocompetent gentleman underwent biopsy for an indurated purplish eyelid and facial lesion with a nodulo-ulcerative forehead component [Figure 1]a. On histopathology, infiltration of skin and muscle with atypical spindle cells having pleomorphic nuclei and scant cytoplasm [Figure 1]b that stained positive for CD31 [Figure 1]c and CD34, with a high Ki-67 index, were noted suggestive of cutaneous angiosarcoma. The patient received external beam radiotherapy and systemic chemotherapy [Figure 1]d. Angiosarcoma is a cancer of the blood vessels of the skin giving a bruise-like appearance.
