Anterior pyramidal polar cataract in an adult

A 46-year-old female presented with bilateral anterior pyramidal polar cataract [Figure 1]a which has previously been reported only in children. Best-corrected visual acuity was positive light perception but inaccurate projection of rays, in the right eye and 20/200 in the left eye. Ultrasound biomicroscopy confirmed the findings [Figure 1]b.

She underwent phacoemulsification with intraocular lens implantation in the left eye. Her postoperative best-corrected visual acuity was 20/30. The right eye however was amblyopic in concordance with previous reports.
This case is shared because of its unusual presentation in adulthood.
